Preparing to File Taxes - Form 1099-SA and more

People often have questions about what is needed to file taxes when they have contributed to an HSA. Here is a list of documents you will need to complete your taxes. 

  • Your W-2 – Provided by your employer
  • Form 8889 – HSA tax form
  • Your tax form (for example, Form 1040)
  • Form 1099-SA – Summary of distributions from your HSA; sent from your HSA custodian, generally by the end of January or beginning of February

You will also receive Form 5498-SA which lists prior year contributions. The 5498-SA is issued by your HSA custodian by May 31st. It is for informational purposes only and not required for tax filing.

All contributions deposited into your HSA through payroll will be listed on your W-2, but if you need access to your contributions for the prior year, you can find that information through the Tango site.  Log in to Tango here: Tango Health Login and click on the “HSA” tab in the blue menu bar. In the sub menu click “Details.” This screen displays your prior year contributions as well as your current year contributions.  

Keep in mind, you have until the deadline for filing your individual tax return (generally April 15th) minus extensions to contribute for the previous year.  

In addition, here are instructions on how to access your tax documents through each custodian bank:

 

Optum Bank:

Optum Bank mails 1099-SA forms out at the end of January. It should also be available online as well, once it has been created. The online version will be available if you log in to your account at the bank's website:

  1. Log into the Optum Bank website: https://www.optumbank.com/

  2. Once you are logged in, click "Help & Tools", then "Forms & Documents" in the dropdown box.

  3. Click on the tax document you want for the desired year.

HSA Bank:

HSA Bank mails 1099-SA forms out at the end of January. It should also be available online as well, once it has been created. The online version will be available if you log in to your account at the bank's website:

  1. Log in to the HSA Bank website:  HSA Bank  
  2. Once you gain access to your HSA Bank account, click on "Tax documents" under the "E-Statements."
  3. Click the needed tax year to download into a PDF form
  4. Print form if needed

UMB Bank: 

Typically the 1099-SA's are mailed out the last week in January. If you do not receive your 1099-SA, you should call UMB, confirm the address on file, and request that a copy of your 1099-SA be resent.

Per UMB's automated line, tax forms are available on the online portal in the Tax Summary section.
